Services for Those with Physical Disabilities
Stearns County Human Services provides care coordination services for children and adults with physical disabilities and adults with traumatic brain injury (Children and adolescents with a traumatic brain injury may be eligible for Developmental Disability services (1085.htm) ). The care coordinator assists persons to gain access to needed supports and services such as home health care, housing supports, employment supports, etc.
To qualify for county services, persons need to be eligible for the Community Alternative Care (CAC), Community Alternatives for Disabled Individuals (CADI), or Traumatic Brain Injury (TBI) programs. Eligibility for these programs includes:
Community Alternative Care (CAC)
• Under age 65
• A resident of an acute care hospital or at risk of frequent or prolonged chronic inpatient hospital care
• Certified disabled
• Eligible for Medical Assistance
Community Alternatives for Disabled Individuals (CADI)
• Under age 65
• Without home and community-based services, the person would require nursing facility level of care
• Certified disabled
• Eligible for Medical Assistance
Traumatic Brain Injury (TBI)
• Under age 65
• Diagnosed traumatic or acquired brain injury
• Without home and community-based services, the person would require nursing facility or neurobehavioral hospital` level of care
• Certified disabled
• Eligible for Medical Assistance
Personal Care Assistant (PCA) Assessments
Stearns County Human Services public health nursing staff perform Personal Care Assistant (PCA) assessments. PCA is a service for persons with disabilities eligible for Medical Assistance to assist them with daily living activities. Public health nurses determine the amount of PCA services to be provided.
