Medical Assistance Health Care

Applying for Health Care Programs

To apply for any of the Minnesota Health Care programs, you will need to file a Minnesota Health Care Application in the county in which you reside. After you've completed the application, you may deliver it to the County by mail or in person. If you want to apply for MinnesotaCare only, you can send your application directly to:

The Minnesota Health Care Program Application is available at the Department of Human Services web site (see links below), or you may call 320.656.6000 and ask for an application to be mailed to you.

Download the form in English, Somali, Spanish, or Vietnamese.  The DHS web site also has links to the form in other languages.

MinnesotaCare

This program is for people who are not eligible for Medical Assistance for General Assistance Medical Care, do not have Medicare or access to employer subsidized health insurance.  There is a monthly premium, and coverage starts the month after you pay your premium.

How to Apply: 

You may obtain a Combined Application Form (CAF) by making a request in one of the following ways:

  • Mail your request to Stearns County Human Services, PO Box 1107, St. Cloud, MN 56302-1107
  • Call Stearns County Human Services at 320.656.6000 or 1.800.450.3663
  • Stop by the Stearns County Human Services office at 705 Courthouse Square, St. Cloud (the Administration Center) on the second floor at the “front desk”
  • Stop by the Melrose branch office of Stearns County Human Services at 114 South 1st Avenue West, Melrose
