HIPAA Rules
Effective April 2003, certain pieces of health
information have additional regulations regarding their collection,
storage, use, and distribution. These changes are required by the
Federal Health Insurance Portability and Accountability Act (HIPAA).
As required by this act, this set of policies

your rights regarding data, and restrictions regarding uses of
protected health information.
The Notice of Privacy Practices
(106 Kb Adobe Acrobat file) is a short description of how medical
information about you may be used and disclosed, tells you how you can
get access to such information, and tells you how to file a complaint
or report a problem.
